Job Description:

Role Purpose: Under the supervision of a RN or LPN, provides quality patient care in a safe and efficient manner.

• Receives shift report from RN & CNA. Regularly completes walking rounds with previous shift.
• Immediately greets and orients patients/families upon arrival to the floor assuring exception customer service.
• Accurately collects and documents data in a consistent manner for the admission assessment and re-assessment at specific times for patient population.
• Communicates effectively with RN and/or LPN related to the changes in the condition and abnormal findings in a timely manner.
• Ensures that patients' ADL's and assigned duties are completed and accurately documented within the time frame assigned.
• Communicates effectively with customers by providing information for patients and families on a regular basis.
• Consistently rounds on all assigned patients according to protocol.
• Routinely prepares patients for discharge and accompanies them upon dismissal.
• Answers call lights appropriately within a timely manner.
• Attends 100% mandatory meetings, 50% of departmental unit meetings and reads 100% of minutes from meetings not attended.

Job Requirements:

Minimum Qualifications:
• High School diploma or equivalent
• State of Illinois CNA license
• BLS certification

Physical Requirements: Employees in this position are, or can be, required to possess the following physical capabilities:
• Lifting, carrying, pushing and/or pulling up to 100 lbs.
• Stooping, kneeling and/or crouching daily
• Standing or walking more than 12 hours a day
• Running to respond to patient emergencies
• Reaching, grasping, fingering, and/or feeling more than 12 hours a day
• Visual acuity in order to correctly distinguish skin tone variations, evidence of IV infiltration, infection; able to read telemetry, blood pressure gauges, etc.
• Audio acuity in order to correctly distinguish various body sounds (i.e., respirations, heart tones, bowel sounds).
• Audio acuity to discern sounds made by various types of machinery or equipment that would indicate dangerous or improper operation.
