Job Description:

Role Purpose: Under general supervision of BHS Central Intake Director, completes intake and assessment functions necessary to arrive at appropriate level of care determinations for ED, medical floor, and referral source patients in need of mental health care.

• Ensures that all Behavioral Health patients in need of acute intervention receive an appropriate level of care through performance of face-to-face/telemed assessments, in emergency departments, medical units, and in outpatient areas throughout SSM Health, Illinois Region
• Ensures that patients who attempt to access Behavioral Health Care through the Regional Operations Center are assessed for appropriate admission to Behavioral Health Unit
• Ensures satisfaction with services by working closely with physicians, referral sources, and internal customers to achieve patient dispositions.
• Promotes exceptional patient safety through the careful application of evidenced-based practice to determine level of risk around suicide, self-harm, and violence parameters.
• Conducts group therapy as needed
• Provide crisis intervention as needed
• Applies DSM and mental health treatment principles in the provision of crisis intervention and family and patient education regarding disease processes
• Documents clinical and disposition information in approved medical format into electronic medical record and into Central Intake database

Job Requirements:

Minimum Qualifications:
• Require a Master degree in a human service discipline ( e.g., social work, counseling, marriage and family therapy, public health, education and or health science) OR Bachelor of Science Degree in Nursing with a minimum of one year experience in a mental health setting
• Current License social Worker, License Professional Counselor, or be license-eligible
• Excellent communication and assessment skills and good judgment related to risk assessment of patients with mental illness.
• Working knowledge of major DSM diagnostic criteria.

Preferred Qualifications:
• 1-3 years’ clinical experience in mental health setting. One year leadership experience preferred
Physical Requirements:

Employees in this position are, or can be, required to possess the following physical capabilities:
• Standing or walking more than 8 hours a day
• Running to respond to patient emergencies
• Visual acuity in order to correctly distinguish skin tone variations, evidence of IV infiltration, infection; able to read telemetry, blood pressure gauges, etc.
• Audio acuity in order to correctly distinguish various body sounds (i.e., respirations, heart tones, bowel sounds).
